The personal information you provide using this form is to allow us to:
a) complete our internal parish registers;
b) provide notification of your reception of the Sacrament(s) to the Archdiocese of Montreal; and/or
c) notifiy the parish of your baptism that you have received further Sacrament(s) with Saint Willibrord's.
The information recorded in our parish registers is our official record of your pastoral history, and is kept confidential.
You may attach your applicable documents here if you have digital copies (photos are acceptable as long as they are clear and legible). You may arrange with the Formation Coordinator to provide hard copies instead of digital ones; please indicate this in "More Info" if you're not attaching your documents to this form.
If you do not have access to a copy of your Birth Certificate, please provide your place of birth and your parents' full names (including your mother's maiden name) in "More Info."
If you were previously received into another Christian denomination but do not have access to any written confirmation of it, please use "More Info" to indicate this and to provide as much information as you are able.
